What is it that this homeless shoeshine man does to his customers that makes the look that tense?

Seriously though, this young woman has had a long conversation with the homeless man and it seems she was more interested in his story than in getting a shoe shine.

I’ve never imagine shoe shine to be a painful experience, but you would think it is a torture when you look at the man receiving it.
